Prerequisites
- Your child has a disability or is at risk of becoming disabled.
- Your child has already celebrated his or her 3rd birthday.
- Your child is not yet enrolled in school
Documents required
- Completed and signed application for early intervention (see link form: Application for integration assistance for disabled children or children at risk of disability)
For new applications additionally:
- Copy of parents’ identity cards/passports
- Report from the Youth Psychiatric Service or the Counselling Centre North
- Proof of need (only if care lasts more than 6 hours and if the need is not included in the report)
For follow-up applications additionally:
- In case of educational need: current proof of need either from the General Social Service (ASD) or the daycare center
- if possible a development report of the daycare center
In case of changes to an ongoing case,
- When changing daycare: confirmation of termination from the daycare
- If the care requirement changes (more than 6 hours per day): proof of need
Please note
In all correspondence please always state the following:
- Child’s last name
- Child's first name
- Date of birth of the child
- Complete registration address of the child
Deadlines
You can apply for early intervention at least six months before the start of care. Early intervention can only be approved once the application has been received by the responsible authority.
You should submit a follow-up application to the responsible authority at least three months before the end of the approval period. A follow-up application can be re-approved no later than the first of the month in which the application was made.
Applications are usually approved for a maximum of one year, unless the need is shorter than one year.
