Prerequisites
- Your child is older than 3 years and not yet enrolled in school.
- There is an assessment from one of the following bodies, by which your child has been assigned to the eligible group of people and classified in a group requiring assistance:
- for physically disabled children: the responsible health authority or the state doctor for physically disabled people,
- for blind and significantly visually impaired children: the state doctor for the visually impaired,
- for hearing-impaired children: the state doctor for hearing-impaired children,
- for children with speech impairments: the state doctor for speech impairments,
- for mentally and emotionally disabled children: the responsible health authority,
- for children with multiple disabilities (especially mental or emotional disabilities): the responsible health authority
- in the case of children with multiple disabilities (especially physical disabilities) or the state doctor for physically disabled people

- Application for integration assistance for disabled children and children at risk of disability,
- official medical report,
- birth certificate,
- Registration confirmation
If your child requires more than 8 hours of care per day, you must submit additional documentation to prove your activity. This includes, for example, your employment or training contract, proof of self-employment, or similar.

Deadlines
Submit your application at least 3 months before the start of childcare or 3 months before your current daycare voucher expires.
The earliest date of approval is the date of the expert opinion, provided this is prepared after the application has been received. If the application is submitted after the expert opinion has been prepared, the earliest start date of approval is the date of receipt of the application.
